Sweet Dreams Anesthesia Forms
Sweet Dreams Anesthesia is a third party service Lone Peak Dental uses for in-office sedation. Their CRNAs are board certified and provide the highest standard of anesthesia care. If your appointment requires sedation, please be sure to fill out the forms and bring them with you to your appointment. Feel free to contact our office if you have any additional questions.
