Scheduling Policy -
Lone Peak Dental is committed to providing all of our patients with exceptional care, and your appointments are very important to us. Scheduling allows us to reserve an appointment time specifically for you. Please understand that when you forget or cancel your appointment without giving enough notice, we miss the opportunity to fill that appointment time, and other patients on our waiting list miss the opportunity to receive services. We politely ask that you reschedule at least 48 hours prior to your scheduled appointment time.
As a courtesy reminder, our appointments are confirmed 48 hours in advance. Please call our office to reschedule your appointment as soon as possible. If we have enough notice, we will be able to schedule you another appointment. Our voicemail service is also available 24/7, so you can leave a message at any time. We appreciate your cooperation in this matter. This policy ensures efficient scheduling, which benefits all parties involved.
Financial Policy -
As a courtesy, we will submit a claim of the dental services rendered to your insurance company. It is your responsibility to be educated on your individual plan and understand the benefits it provides. Our office policy is to collect your estimated portion on the date that your services are rendered.
We also offer Care Credit as a third party financing option, which has a 12 month interest free payment plan. For more information about Care Credit please visit www.carecredit.com.

-Delta Dental (Premier and PPO)
-Dental Select (Platinum only)
-EMI (Premier and Advantage)
*We also accept Utah Medicaid for children under the age of 12.
